What Is Form DHA-335?
Form DHA-335 is the official Department of Home Affairs form for reporting a lost or stolen South African passport or travel document. Submitting this form is a mandatory first step before DHA will process a replacement passport application.
The form cancels the lost or stolen document in DHA’s systems, preventing it from being used fraudulently. A replacement passport cannot be issued until the original is formally reported lost or stolen.
When Do You Need Form DHA-335?
You need Form DHA-335 when:
- Your South African passport has been lost
- Your passport has been stolen
- Your passport has been damaged beyond use and requires replacement
- Your travel document (emergency travel certificate or official passport) has been lost or stolen
If your passport was stolen: Report the theft to the police before completing DHA-335. You will need a police case number to include on the form.
How to Complete Form DHA-335
Complete in black ink, block letters.
Personal details: Your full name, ID number, date of birth, and contact details.
Details of the lost or stolen document: Passport number (if known), date of issue, where it was issued, and the date it expires or expired.
Circumstances of loss: A clear description of how the passport was lost or stolen — where it happened, when you discovered it was missing, and any relevant details.
Police case number: If the passport was stolen, include the case number from the police report. If it was genuinely lost (not stolen), a police report may still be required by some DHA offices — confirm this with your local branch before your appointment.
Declaration: Sign in the presence of the DHA official receiving your application.
Documents to Bring With Your DHA-335
- Completed Form DHA-335
- Police report (always required for stolen passports; required by most branches for lost passports)
- Original South African ID (Smart ID or green ID book)
- Certified copy of your ID
- Completed Form DHA-73 (passport application form) — to apply for the replacement immediately
DHA-335 is submitted together with DHA-73 at the same appointment. You report the loss and apply for the replacement in a single visit.

The double tariff applies specifically to lost and stolen passports. A damaged passport where the loss was not through negligence may qualify for the standard tariff — confirm with the DHA official at the time of application.
If Your Passport Was Lost or Stolen Abroad
If you are outside South Africa and your passport has been lost or stolen:
- Report the theft or loss to the local police and obtain a police report
- Contact your nearest South African embassy or consulate immediately
- Complete Form DHA-335 at the mission
- Complete Form DHA-73 for a replacement or emergency travel document
- The mission can process an emergency travel certificate to get you home if your passport cannot be replaced quickly

Frequently Asked Questions
My passport just expired — do I still need DHA-335? No. An expired passport is a normal renewal, not a loss. Use Form DHA-73 only, and bring your expired passport as part of the renewal application. DHA-335 is only for lost, stolen, or damaged documents.
I lost my passport years ago and never reported it — is it too late? It is never too late to report a lost passport. Submit DHA-335 as part of your replacement application at any time. DHA will cancel the old document in the system when you report it, regardless of how long ago it was lost.
Can someone use my lost passport fraudulently? Reporting it using DHA-335 causes the passport to be flagged as cancelled in the Home Affairs Travel Document System and at border control. Until it is reported, the document technically remains active in the system. Report it as soon as you discover it is missing.
Will I be penalised for losing my passport? Not criminally. The double tariff (R1 200) is the financial consequence. DHA does not impose travel bans or penalties beyond the increased fee.
