What Is Form DHA-196?

Form DHA-196 is the official Department of Home Affairs application form for an adult (18 years or older) who wants to change their surname. It is also referred to as BI-196.

The form covers surname changes in several circumstances: after marriage or divorce, reverting to a birth surname, assuming a stepfather’s or adoptive parent’s surname, or changing to a completely different surname by application.

For forename (first name) changes, use Form DHA-85 instead. For surname changes for children (under 18), use Form DHA-193 instead.


When Do You Need Form DHA-196?

You need Form DHA-196 when you are an adult (18 or older) wanting to:

  • Assume your spouse’s surname after marriage
  • Revert to your birth surname after divorce
  • Assume any of your previous surnames
  • Take on a stepfather’s or adoptive parent’s surname
  • Change to a different surname by formal application

Getting married: When you marry in South Africa, your marriage registration automatically creates a record of the name change at DHA. You still need to complete DHA-196 to have your ID and other documents reissued in the new surname.


How to Complete Form DHA-196

Complete in black ink, block letters. Do not use correction fluid.

Section A — Applicant details Your current full name exactly as it appears on your ID, your ID number, date of birth, and contact details.

Section B — Surname you wish to assume The new surname you want to use. If you are reverting to a birth surname, write that surname here. If taking a spouse’s surname, write the spouse’s surname.

Section C — Reason for change Tick the applicable reason: marriage, divorce, reversion to birth surname, adoption, or other. For “other” reasons, provide a written explanation.

Section D — Supporting documentation checklist Confirm that you are attaching the required supporting documents. Tick each item you are including.

Section E — Declaration Sign in the presence of the DHA official receiving your application.


Documents to Bring With Your DHA-196

All applications:

  • Completed Form DHA-196
  • Original South African ID (Smart ID or green ID book)
  • Certified copy of your ID

Surname change after marriage:

  • Original marriage certificate (or certified copy if the original is not available)
  • Certified copy of marriage certificate

Reversion to birth surname after divorce:

  • Original divorce decree (or certified copy)
  • Original birth certificate showing your birth surname
  • Certified copies of both documents

Adoption or stepparent surname:

  • Original adoption order or court order
  • Certified copy of the order

Other surname changes:

  • Supporting affidavit explaining the reason for the change
  • Any documentary evidence supporting the application (court orders, deed poll, etc.)

The R140 fee for a married applicant covers both the applicant and their spouse, plus any minor children. Separate fees are not charged per family member in this case.


What Happens After You Submit DHA-196?

Once DHA processes the surname change:

  1. Your records in the National Population Register are updated
  2. Your existing ID is cancelled
  3. A new Smart ID card is issued in your new surname
  4. You will receive an SMS when your new ID is ready for collection

Processing typically takes 2 to 6 weeks. During this time, your old ID remains valid. Once you receive the new ID, the old one must be surrendered.

You will then need to update your surname on other documents separately: driver’s licence (DLTC), SARS, bank accounts, medical aid, UIF, and pension records.


Common Mistakes on DHA-196

Using the wrong form. DHA-196 is for surname changes for adults. First name changes require DHA-85. Minor surname changes require DHA-193.

Not bringing the original supporting document. Certified copies alone are not sufficient. DHA requires the original document (marriage certificate, divorce decree, etc.) to verify the copy. The original is usually returned to you.

Incomplete reason for change. If ticking “other” in Section C without a clear explanation, the application may be referred for review or rejected.


Frequently Asked Questions

Can I change my surname to anything I want? Not freely. DHA applies the provisions of the Births, Marriages and Deaths Registration Act. Changes must be based on one of the recognised grounds. Random or preference-based surname changes are not automatically approved and may require a formal application with supporting motivation.

Do I need to change my surname on my passport separately? Yes. A surname change on your ID does not automatically update your passport. Once your new ID is issued, apply for a new passport using Form DHA-73. You can use your old passport (with your old name) alongside your new ID (with your new name) during the transition period.

Can I revert to my birth surname without being divorced? Not through DHA-196 — reversion is specifically linked to divorce or legal separation under this form. If you wish to use a previous surname for other reasons, speak to a DHA official about the applicable process.

Does my spouse also need to submit DHA-196? Only if they are also changing their surname. The R140 fee covers both spouses, but each person whose surname is changing must complete their own DHA-196 form.
