What Is Form DHA-1682?

Form DHA-1682 is the Department of Home Affairs application form for inserting a natural (biological) father’s particulars into the birth register of a child where the father’s details were not recorded at the time of the original birth registration.

This is typically used when a child was registered under the mother’s surname with no father listed, and the biological father later wants to be officially recorded — with both parents’ consent.


When Do You Need Form DHA-1682?

Use Form DHA-1682 when:

  • A child was registered at birth without the father’s details being recorded
  • The biological father wants to formally acknowledge paternity and have his details added
  • Both parents consent to the insertion of the father’s particulars
  • The child may also need to be registered under the father’s surname as part of this process

Requirements

Both parents must be present and consent to the insertion of the father’s particulars. DHA will not process this application if only one parent attends — the mutual consent requirement protects both the child and the father.

The father must be a willing party. This process cannot be used to force paternity acknowledgement on an unwilling father. Court proceedings are required if there is a paternity dispute.

  • If the parents have since married: marriage certificate (original and certified copy)

What Happens After Submission?

Once DHA processes the application:

  • The father’s details are added to the child’s birth register
  • A new birth certificate is issued reflecting the updated registration
  • If the surname has also been changed, this is reflected in the new certificate

Frequently Asked Questions

Can I do this if the father is not cooperating? No. DHA-1682 requires both parents to consent and be present. If the father refuses to acknowledge paternity or is uncontactable, this is a legal matter. You may need to pursue a court order for paternity and use that as the basis for amending the register.

What if the father is foreign? The process is the same. The father must be present with his valid passport and any applicable South African documentation.

Does adding the father’s name change the child’s surname automatically? No. The insertion of the father’s particulars records his details but does not automatically change the child’s surname. A separate surname change request (with both parents’ consent) must be made — this can be done at the same time.
