The Complete DHA Forms List

Every application at the Department of Home Affairs requires at least one form. This page lists all 25 core DHA forms by service category, with a plain-English explanation of what each form does, when you need it, and whether it can be downloaded.

Before you download anything: confirm the form number with the guide for your specific service below. Some processes require multiple forms, and some forms have been renumbered or updated since earlier versions circulated online.

Download only from dha.gov.za. Third-party copies are frequently outdated. Each form page on this site links directly to the official DHA source.


Rules That Apply to Every DHA Form

  • Complete in block letters using black ink only
  • Do not use correction fluid — a corrected form will be rejected; collect a new one
  • Do not pre-sign — most forms must be signed in the presence of a DHA official
  • Bring the original form to your appointment — photocopies are not accepted for submission
  • Keep a certified copy of every form after it has been stamped

The following forms contain unique barcodes assigned to each individual application. They must be collected in person at a DHA branch office or, for applicants overseas, from a South African embassy or consulate.

Important Warnings

Do not use third-party form websites. Sites offering to “fill in your DHA form online” for a fee are not affiliated with the Department of Home Affairs. DHA does not authorise any third party to charge for form completion assistance. Forms submitted through unofficial channels may be rejected or — in some cases — used for identity fraud.

Do not sign a form before your appointment. DHA forms that require a signature must be signed in the presence of the official receiving your application. Pre-signed forms may be technically invalid and some officials may reject them.

Forms change without notice. DHA updates form versions periodically. Always download a fresh copy before your appointment — do not use a form printed months earlier. If a DHA official rejects your form as an outdated version, they should provide the current version at the branch.
